If you are wanting to submit a Shy Bladder Log after you have completed your mock test, you can submit it along with your forms via the upload module in the Mock Testing section of your course. If you are having issues uploading your forms that way, you can always email them directly to support@datac.ca.

Following the completion of a real specimen collection, the Shy Bladder Log must be sent to the Medical Review Officer (MRO) and the lab along with the lab copy of the CCF.
